We’re looking for a Senior Community Health Worker, someone who’s ready to grow with our company. In this position, you’ll be responsible for supporting patients and families in accessing community programs and resources to promote healthy outcomes and well-being. The Senior Community Health Worker is a vital part of the care and will assist patients and families with applications for public benefits and community programs; educate families on healthy behaviors and community resources; and be an advocate for the patient and family.

Job Duties & Responsibilities
Screens patients and families for social drivers of health
Assists families in identifying and meeting goals to promote health and well-being
Identifies appropriate community resources and submits referrals for families
Provides education to families on community resources, healthy behaviors, and healthcare navigation
Assists families in accessing community programs and medical services
Assists patients and families with completing applications for public benefits assistance including SNAP, Medicaid, TANF, and SSI
Follows up with patients and addresses barriers to accessing community programs and medical services
Manages an assigned caseload of patients
Demonstrates empathy and provides emotional support to patients and families
Acts as a patient advocate and liaison between the patient/family, community service agencies, and the clinical care team
Documents clinical encounters in an electronic medical record and tracks outcomes
Builds trusting relationships with patients, families, clinical team members, and community partners
Collaborates with other members of the care team
Develops and maintains key community partnerships
Continuously expands knowledge and understanding of community resources and services
Attends department meetings
Completes required trainings
Skills & Requirements
H.S. Diploma or GED required
Associate's degree Health Related field preferred
CHW - Community Health Worker by the Texas Department of State Health Services (DSHS) required
5 years Healthcare, nonprofit organization, government agency, or organization that provides direct assistance to individuals preferred
